You can help us by providing important information about yourself and your health. Before your first appointment, please complete the following forms and bring them with you.

Medical records, x-rays, or other information that can help our surgeons understand your medical conditions
A list of any medications you take
A list of your questions and concerns
Your insurance card (please bring this to every visit)

Please arrive 15 minutes before your scheduled appointment time. You’ll receive a phone call one business day before your appointment as a reminder.

If you must cancel an appointment, kindly give us 24 hours notice so that we may accommodate another patient with urgent needs. A charge will be incurred for failure to cancel a schedule appointment.
